Policy – District annually distributes standardized immunization document from Colorado Department of Public Health and Environment to parents listing required and recommended immunizations. Students must submit current immunization certificate, written authorization for health officials to administer immunizations, or valid exemption. Students without proper documentation face suspension/expulsion. All distribution materials inform parents of exemption rights for health, religious, personal, or other reasons. Immunization and exemption rates publicly available upon request.
