- If your child has any medications at school they must be picked up by the end of the last day of school (Thursday May 25th). Students will not be allowed to transport medications home. Any medications not picked up will be disposed of per protocol.
- All students entering 6th grade in the Fall must get the Tdap immunization and record of this must be turned into the school nurse before the first day of school. Records can be turned in to the school office.
- All Preschool students advancing to Kindergarten in the Fall must have their immunizations up to date to include Hepatitis B series of 3 shots, Dtap series of 4 or 5 shots with final one being after age 4, Polio series of 3 or 4 shots, MMR series of 2 shots, and Varicella series of 2 shots.
- If your child receives any immunizations over the summer, please be sure to provide a copy of the record to the school nurse. Records can be turned in to the school office.
